Shift Type: Monday-Friday (Day), 5x8-hour schedule; primary listed time 07:30 AM - 04:00 PM (Day). Shift times may vary and could be one of: 07:00 AM - 03:30 PM, 07:30 AM - 04:00 PM, 08:00 AM - 04:30 PM, or 08:30 AM - 05:00 PM. Weekend coverage required: every other weekend or 4 weekend days in a 4-week schedule.

Roles & Responsibilities

• Coordinate and manage patient discharge planning to ensure smooth transitions of care
• Conduct utilization review and case management activities using Milliman guidelines when applicable
• Document and maintain accurate patient records in Epic to support continuity of care
• Collaborate with interdisciplinary teams (PT, RT, pharmacy, social services, etc.) to create comprehensive plans of care
• Perform needs assessments including DME orders and follow-up care coordination
• Support patient and family education around post-discharge plans and community resources
• Float within the case management service line as needed to cover gaps and maintain coverage

Qualifications

• Active AK Registered Nurse (RN) license in hand required for offer
• Minimum 1 year acute care nursing experience
• Epic EMR experience required (Epic hardstop may apply)
• BLS certification required
• ACM (case management) certification preferred
• Experience with discharge planning, care coordination, and utilization review
• Willingness to work required weekend coverage and float within scope

Education

• Not specified - active AK RN license required (clinical nursing education such as diploma, ADN or BSN typical for RN roles)

Knowledge & Skills

• Proficient with Epic charting and electronic documentation
• Familiarity with Milliman guidelines and utilization review processes
• Strong care coordination and discharge planning skills
• Comfortable collaborating across departments and disciplines
• Basic computer proficiency: Microsoft Office and Teams (preferred)
• Knowledge of CMS regulations and HIPAA best practices in acute care settings
