Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:00 AM - 3:30 PM (Day). Actual start/end may be one of: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00 PM. Weekend coverage required: every other weekend or 4 weekend days in a 4-week schedule.
Hours per Week: 40
Candidate Pay Rate: $66 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum, ensuring smooth transitions and clear plans
- Conduct utilization review and case management activities using Milliman Guidelines
- Develop and implement discharge plans and arrange DME and community resources
- Document accurately and timely in Epic to support quality care and compliance
- Collaborate with interdisciplinary teams including PT, RT, pharmacy and social services
- Perform needs assessments and help set realistic, patient-centered plans of care
- Float within case management services as needed to cover gaps and support the unit
- Support regulatory compliance with CMS and HIPAA guidelines through thorough charting and communication
Qualifications
- AK RN license in hand required for auto offer (screen for pending licenses)
- BLS required
- ACM (case management) certification preferred
- Minimum 1 year of acute care case management or related acute care experience
- Epic (EMR) experience required
- Willingness to work every-other-weekend or 4 weekends per 4-week schedule
- Must be at least 50 miles from the facility (locals not accepted); first-time travelers accepted
Knowledge & Skills
- Strong knowledge of case management and utilization review processes
- Comfortable using Epic for charting and care coordination
- Familiarity with Milliman Guidelines and discharge planning best practices
- Skilled at care coordination, interdisciplinary communication, and patient advocacy
- Proficient with Microsoft Office and Teams for team communication and documentation
- Ability to manage a varied adult and pediatric patient population across acuity levels
