Roles & Responsibilities

• Perform thorough evaluations and develop individualized treatment plans for SNF residents
• Provide skilled therapeutic interventions to improve mobility, strength, and safety
• Document patient progress clearly and timely in accordance with facility and regulatory standards
• Collaborate with nursing, case management, and families to coordinate comprehensive care
• Assist with discharge planning and recommendations to support safe transitions
• Participate in weekly treatment team meetings and contribute clinical expertise
• Maintain professional standards, including infection control and patient safety protocols

Qualifications

• Current, unrestricted Physical Therapist (PT) license — New York licensure required (client will not wait for licensure)
• Experience working in a Skilled Nursing Facility (SNF) setting is required; new grads and assistants will not be considered
• Active BLS/CPR certification
• Strong documentation skills and familiarity with weekly per-cycle documentation
• Ability to float within a 25-mile radius and cover neighboring facilities as needed
• Reliable transportation and willingness to travel locally during the assignment

Education

• Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy (or equivalent)
• Relevant state PT licensure and required certifications

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait training, and fall prevention
• Knowledge of SNF regulations, billing cycles (weekly), and rehab documentation standards
• Proficient in creating measurable goals and tracking outcomes
• Excellent communication and interdisciplinary teamwork skills
• Time management and prioritization to manage a full caseload effectively
