Shift Type: Monday-Friday, Day shift (5x8) with start times varying between 7:00 AM - 8:30 AM and end times between 3:30 PM - 5:00 PM (typical options: 7:00 AM - 3:30 PM; 7:30 AM - 4:00 PM; 8:00 AM - 4:30 PM; 8:30 AM - 5:00 PM). Day shift with required weekend coverage (every other weekend or 4 weekend days in a 4-week schedule)
Hours per Week: 40
Candidate Pay Rate: $66 per hour

Roles & Responsibilities
- Coordinate comprehensive discharge planning and care transitions to ensure safe, timely patient discharges
- Conduct utilization review and apply Milliman guidelines to support appropriate level-of-care decisions
- Develop and manage individualized plans of care, including needs assessment and ordering DME when required
- Collaborate with interdisciplinary teams (PT/OT, pharmacy, social services, etc.) to close care gaps
- Document thoroughly in Epic to meet regulatory and facility standards
- Communicate with patients, families, and community providers to facilitate continuity of care
- Float within case management services to cover coverage gaps and support unit needs
Qualifications
- AK RN license in hand required for Auto Offer (screen for pending license; license must be active)
- Minimum 1 year acute care / acute hospital experience required
- BLS certification required
- Epic EMR experience required (Epic hardstop requirement noted)
- ACM certification preferred but not mandatory
- Ability to perform utilization review and case management duties (discharge planning, care coordination, Milliman guidelines)
- Willingness to work every-other-weekend rotation and float within scope to meet facility needs
- First-time travel clinicians welcome; candidate permanent address must be at least 50 miles from the facility (locals not accepted)
Knowledge & Skills
- Proficiency with Epic charting and documentation workflows
- Strong knowledge of care coordination, discharge planning, and Plan of Care development
- Familiarity with Milliman guidelines, CMS regulations, and HIPAA requirements
- Experience with utilization review and needs assessment / ordering DME
- Comfortable using Microsoft Office and Teams for communication and documentation
- Ability to collaborate with interdisciplinary teams and navigate high-volume acute hospital environments
