Roles & Responsibilities

• Provide compassionate, high-quality patient care and clinical support throughout the shift
• Administer medications and treatments accurately, following facility policies
• Perform thorough patient assessments and report changes promptly to the care team
• Document patient care clearly and timely in the electronic health record
• Collaborate with nurses, therapists, and support staff to ensure coordinated care
• Educate patients and families about care plans, medications, and discharge instructions
• Maintain a safe, clean environment and adhere to infection control standards

Qualifications

• Active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license — LPN preferred as listed
• Current BLS/CPR certification or willingness to obtain upon hire
• Comfort with medication administration, basic assessments, and documentation
• Strong communication and teamwork skills; patient-focused and dependable
• Previous acute care or rehab experience is a plus but not required

Education

• LPN: Vocational nursing diploma or certificate (or equivalent)
• RN (if applicable): Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Solid knowledge of basic nursing care, vital signs, and medication administration
• Experience with patient assessment and reporting clinical changes
• Familiarity with electronic health records and accurate documentation practices
• Ability to prioritize tasks in a fast-paced clinical environment
• Good interpersonal skills for working with patients, families, and multidisciplinary teams
