Roles & Responsibilities
- Provide compassionate, hands-on care to residents, assisting with activities of daily living such as bathing, dressing and mobility
- Monitor residents' condition and report changes promptly to the nursing team to ensure timely interventions
- Deliver medication reminders and assist with basic care tasks following facility protocols
- Document care accurately and clearly to maintain excellent clinical records
- Support a safe, clean and comfortable environment by following infection control and safety procedures
- Collaborate with colleagues and families to create individualized, dignified care plans
Qualifications
- Active LNA license valid for New Hampshire or Vermont (Licensed Nursing Assistant)
- Eligible to work the assigned Monday–Friday, 2:00 PM–10:00 PM schedule
- Current immunizations and compliance with facility onboarding requirements (background check, TB screening) as required by employer
- Previous experience in long-term care or similar clinical setting is a plus
Education
- Completion of an approved Nursing Assistant program or equivalent
- High school diploma or GED preferred
Knowledge & Skills
- Strong patient-centered care skills with an emphasis on respect and dignity
- Comfortable assisting with ADLs and mobility support
- Basic clinical observation skills and the ability to communicate changes in condition
- Good documentation and time-management habits
- Team-oriented attitude and strong interpersonal communication
