Roles & Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver evidence-based therapeutic interventions to improve mobility and function
• Monitor patient progress and adjust treatment plans to achieve optimal outcomes
• Document assessments, plans and progress notes accurately and timely
• Collaborate with interdisciplinary team members to coordinate high-quality care
• Educate patients and caregivers on home exercise programs and injury prevention
• Participate in unit meetings and contribute to quality improvement initiatives

Qualifications

• Licensed Physical Therapist (PT) — ability to practice in New Jersey is required
• Graduate of an accredited physical therapy program (DPT preferred)
• Current BLS/CPR certification (or obtain upon hire)
• Clinical experience in outpatient or post-acute rehabilitation is a plus
• Strong communication, documentation, and patient education skills
• Ability to work Monday through Friday daytime schedule for the duration of the assignment

Education

• Graduate of an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy and functional mobility training
• Proficient with patient assessment, goal-setting and progress measurement
• Familiarity with documentation standards, CPT coding and therapy workflows
• Comfortable collaborating with interdisciplinary teams and family caregivers
• Good time management and caseload prioritization skills
