Roles & Responsibilities
- Provide resident-centered physical therapy services under the supervising therapist’s plan of care to maximize functional ability and independence
- Perform therapeutic procedures and progress treatment programs while monitoring responses and reporting status changes promptly
- Document all therapeutic interactions timely and accurately so the whole care team stays informed
- Recommend, obtain, and instruct residents in appropriate assistive devices to support safety and mobility
- Educate residents and caregivers with clear, compassionate teaching and document learning outcomes
- Collaborate in interdisciplinary team meetings to support coordinated, high-quality care
- Follow department protocols, infection control practices, and Medicare/Medicaid regulations
- Maintain department cleanliness and assist with routine equipment and space upkeep
Qualifications
- Associate degree required (Physical Therapist Assistant or related field preferred)
- PT Assistant License required in the practicing state
- Basic Life Support (BLS) certification required
- Prior experience in long-term care or a skilled nursing facility is preferred
- Strong verbal and written communication skills and excellent documentation abilities
- Ability to lift up to 50 pounds and perform the physical demands of the role (two-person lifts for >50 lbs.)
- Ability to read, write, speak, and understand English well enough to follow instructions and document care
Education
- Associate degree (Physical Therapist Assistant or related field) required
Knowledge & Skills
- Patient care and therapeutic procedures for rehabilitation
- Assistive device selection and training
- Infection control best practices and departmental cleanliness
- Proficient with Microsoft Office for documentation and reporting
- Time management, prioritization, and excellent documentation skills
- Interdisciplinary team collaboration and strong interpersonal skills
- Resident-centered care approach and effective patient/caregiver education
