Roles & Responsibilities

• Evaluate patients using thorough assessment techniques and develop individualized treatment plans
• Deliver evidence-based outpatient physical therapy interventions for orthopedic and neurological conditions
• Perform fall risk assessments and implement prevention strategies to keep patients safe and independent
• Provide post-operative care including total joint replacement and rotator cuff repair recovery plans
• Document all patient care accurately and timely in NextGen EHR
• Meet clinic productivity expectations, including a minimum target of 11 visits per day and 55 visits per week
• Collaborate with case management, referring providers, and the interdisciplinary team to optimize outcomes
• Educate patients and caregivers on home programs, prevention strategies, and progression of care

Qualifications

• Open to new graduates and experienced clinicians alike — we welcome eager learners
• Experience or strong interest in outpatient physical therapy, especially orthopedic and neurological populations
• Comfortable working toward productivity targets and documenting in NextGen EHR
• Skills caring for adults, geriatric, and adolescent patients
• Knowledge or experience with vestibular rehab, concussion management, Parkinson's disease care, and post-operative rehabilitation
• Professionalism and ability to work effectively as part of a busy outpatient team
• Active state Physical Therapist license or eligibility to obtain state licensure (discuss specifics with recruiter)

Knowledge & Skills

• Outpatient Physical Therapy best practices and clinical reasoning
• Evaluation and assessment skills including fall risk assessments
• Orthopedic management: ACL, meniscus, rotator cuff, total joint replacement and related post-op care
• Neurological management: concussion, vestibular/vertigo, Parkinson’s disease
• Comfort with productivity expectations and visit-based workflows
• Proficiency documenting in NextGen EHR and working with case management processes
• Patient education, care coordination, and strong communication skills
