Roles & Responsibilities
- Perform resident evaluations and develop individualized therapeutic plans of care with meaningful long- and short-term goals
- Deliver therapeutic interventions and monitor progress, adjusting plans to meet functional goals
- Document all evaluative and therapeutic interactions clearly in the Electronic Medical Record for team continuity
- Provide patient and caregiver education, ensuring understanding and safe use of assistive devices
- Consult with physicians and the interdisciplinary team about plan of care and discharge needs
- Recommend, obtain, and instruct on appropriate assistive devices to support resident independence
- Supervise Physical Therapist Assistants and mentor new staff during orientation
- Lead in-service education and help develop new rehabilitation programs to enhance resident care
- Maintain department cleanliness and follow infection control procedures to protect residents and staff
- Ensure documentation aligns with Medicare, Medicaid, and other reimbursement requirements
Qualifications
- Active PT license in the state of work (must be active prior to start)
- Bachelor's degree required; Master’s or Doctorate preferred
- Basic Life Support (BLS) certification preferred
- Prior experience in long-term care or skilled nursing preferred
- Strong verbal, written, and presentation communication skills
- Demonstrated ability to work collaboratively in an interdisciplinary team
- Excellent time management and documentation skills
Education
- Bachelor's degree required; Master’s or Doctorate in Physical Therapy preferred
Knowledge & Skills
- Proficient with Electronic Medical Record documentation and Microsoft Office
- Knowledge of Medicare, Medicaid, and reimbursement documentation practices
- Skilled in evaluative procedures, therapeutic interventions, and assistive device recommendations
- Strong resident-centered care approach and infection control awareness
- Ability to supervise assistant-level staff and provide mentorship
- Excellent interpersonal skills and customer service mindset
