Shift Type: Monday-Friday, 8:00 AM - 4:00 PM Day Shift (day shift typically 8:00 AM - 4:30 PM or until work completed; weekend coverage optional)
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Coordinate timely discharge planning and manage post-acute referrals to ensure smooth patient transitions
- Perform InterQual reviews to support appropriate level-of-care decisions
- Use Midas, NaviHealth and Meditech to document case activity, track authorizations and communicate care plans
- Assess patients for observation versus inpatient status and manage related documentation
- Conduct LTSS screening via VA DMAS as needed to connect patients with long-term support services
- Collaborate with physicians, nurses and interdisciplinary teams to optimize patient outcomes
- Educate patients and families about discharge plans, follow-up care and community resources
Qualifications
- Active RN experience with minimum 2 years of relevant clinical/case management experience
- Demonstrated proficiency in Midas, NaviHealth and Meditech (must-haves for this role)
- Experience with discharge planning and performing InterQual reviews
- Comfort working in inpatient hospital case management covering ICU, PCU and Med Surg floating areas
- Self-motivated, organized, and a team player who completes work in a timely manner
Knowledge & Skills
- Strong knowledge of case management and discharge planning processes
- Experience performing InterQual reviews and documentation best practices
- Familiarity with observation management and status determination
- Ability to perform LTSS screening using VA DMAS resources
- Excellent communication, prioritization and care coordination skills
- Proficiency with Meditech EMR and related care management tools
