Roles & Responsibilities

• Provide compassionate, high-quality rehabilitation nursing care to post-acute patients, supporting their recovery goals
• Perform and document head-to-toe assessments and ongoing telemetry monitoring to keep patients safe
• Administer medications, IV therapy and blood products accurately and timely
• Obtain lab draws and interpret results to inform the care team
• Collaborate with the interdisciplinary team to assist in creating and executing individualized rehabilitation care plans
• Teach and coach patients and families about care plans, discharge goals and self-care techniques
• Use Meditech Magic EMR to document care, communicate with the team and support quality reporting
• Safely assist with patient lifts and mobility, promoting independence while preventing injury

Qualifications

• Active RN license (state-appropriate) — current and in good standing
• Minimum 2 years of relevant nursing experience (post-acute/rehab experience a plus)
• BLS certification — required
• IV certification — required (LPNs must be IV certified; RNs should be proficient with IV therapy)
• Telemetry competency / ability to read telemetry — required
• CRRN certification — preferred
• ACLS certification — preferred (noted as preferred in the posting)
• Experience with blood administration, lab draws, and head-to-toe assessments
• Comfortable using Meditech Magic EMR

Knowledge & Skills

• Telemetry monitoring and rhythm recognition
• IV therapy and medication administration best practices
• Blood product administration and transfusion safety
• Clinical assessment and documentation (head-to-toe assessments)
• Rehabilitation care planning and patient education techniques
• Familiarity with Meditech Magic or similar EMR systems
• Safe patient handling and use of patient lifts
• Quality indicator scoring knowledge in a post-acute setting (preferred)
