Roles & Responsibilities

• Coordinate and manage patient care transitions using evidence-based case management practices
• Lead discharge planning and progression to ensure safe, timely transitions to post-acute settings
• Document care thoroughly and accurately in the EMR (Meditech / Midas)
• Collaborate with physicians, social workers, and interdisciplinary team members to optimize patient outcomes
• Conduct difficult but necessary conversations with patients and families about plans of care
• Monitor throughput and contribute to efficiency efforts to improve patient flow

Qualifications

• Current RN license (ability to practice in Texas; compact license accepted if applicable)
• Minimum 1 year of bedside case management experience (required)
• Experience with Meditech or HCA; familiarity with Midas is a plus
• Knowledge of post-acute care coordination and discharge planning
• Comfortable having difficult conversations and advocating for patients
• Strong communication, organization, and interdisciplinary collaboration skills

Education

• Nursing education: ADN or BSN (BSN preferred where available)
• Active RN licensure appropriate for practice in Texas

Knowledge & Skills

• Expertise in bedside case management and discharge planning
• Proficiency with Meditech and/or HCA systems (Midas experience helpful)
• Understanding of post-acute care options and transitions
• Ability to manage high patient ratios and prioritize effectively
• Strong interpersonal skills for family and MD communications
• Adaptable, proactive approach to problem solving and throughput challenges
