Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day); rotating every other weekend
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Coordinate and manage patient care transitions using evidence-based case management practices
- Lead discharge planning and progression to ensure safe, timely transitions to post-acute settings
- Document care thoroughly and accurately in the EMR (Meditech / Midas)
- Collaborate with physicians, social workers, and interdisciplinary team members to optimize patient outcomes
- Conduct difficult but necessary conversations with patients and families about plans of care
- Monitor throughput and contribute to efficiency efforts to improve patient flow
Qualifications
- Current RN license (ability to practice in Texas; compact license accepted if applicable)
- Minimum 1 year of bedside case management experience (required)
- Experience with Meditech or HCA; familiarity with Midas is a plus
- Knowledge of post-acute care coordination and discharge planning
- Comfortable having difficult conversations and advocating for patients
- Strong communication, organization, and interdisciplinary collaboration skills
Education
- Nursing education: ADN or BSN (BSN preferred where available)
- Active RN licensure appropriate for practice in Texas
Knowledge & Skills
- Expertise in bedside case management and discharge planning
- Proficiency with Meditech and/or HCA systems (Midas experience helpful)
- Understanding of post-acute care options and transitions
- Ability to manage high patient ratios and prioritize effectively
- Strong interpersonal skills for family and MD communications
- Adaptable, proactive approach to problem solving and throughput challenges
