Roles & Responsibilities
- Assess residents’ functional mobility and design individualized treatment plans
- Deliver hands-on therapeutic interventions including gait training and balance work
- Collaborate with nursing and rehab team to coordinate safe, goal-driven care
- Educate residents and families on home programs and fall prevention strategies
- Document progress clearly and timely to support continuity of care and reimbursement
- Adapt interventions to meet the needs of skilled nursing residents and post-acute patients
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent clinical training (new grads welcome)
- Licensed Physical Therapist in New Hampshire or ability to obtain state license promptly
- Valid CPR/BLS certification (or willing to obtain upon hire)
- Comfort working in a skilled nursing/post-acute care setting (SNF)
- Strong communication and documentation skills; team player attitude
Education
- Doctor of Physical Therapy (DPT) preferred
Knowledge & Skills
- Skilled in mobility assessment, therapeutic exercise, and gait/balance training
- Proficient with SNF documentation practices and care-plan collaboration
- Ability to tailor interventions for post-acute and geriatric populations
- Time management and prioritization across a caseload
- Comfortable using electronic medical records and meeting compliance standards
