Roles & Responsibilities
- Evaluate patients and create individualized, goal-driven treatment plans
- Deliver skilled physical therapy interventions to promote safe mobility
- Document clinical progress clearly and timely in the medical record
- Collaborate with nurses, OT, SLP and case managers to optimize patient outcomes
- Educate patients and families on home programs and safe discharge plans
- Contribute to a positive, team-focused culture that prioritizes patient-centered care
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — DPT/ PT credential indicated in the job skills
- New grads welcome — Years of experience: any, including New Grad
- Comfort working in a Skilled Nursing Facility (SNF) practice setting
- Strong communication skills and ability to work collaboratively with interdisciplinary teams
- Current CPR/BLS recommended (confirm facility requirement upon interview)
Education
- DPT or PT degree (Physical Therapy). Job indicates PT / DPT credentials.
Knowledge & Skills
- Skilled in patient assessment, mobility training, and therapeutic exercise
- Knowledge of documentation best practices and care plan development
- Able to perform safe transfers and fall-risk mitigation strategies
- Good clinical reasoning, time management, and patient education skills
- Comfortable working with geriatric and post-acute populations
