Roles & Responsibilities
- Provide skilled occupational therapy services to residents in a skilled nursing facility, focusing on functional independence and quality of life
- Assess patients' strengths, limitations, and goals to develop individualized treatment plans
- Implement evidence-based interventions for ADLs, mobility, cognition, and adaptive equipment needs
- Collaborate closely with nurses, PT, SLP, social work, and nursing leadership to coordinate care and discharge planning
- Thoroughly document evaluations, treatment plans, progress notes, and outcomes in the facility charting system
- Educate residents and families on strategies, home programs, and safety to maximize carryover
- Adapt interventions to each resident’s needs while maintaining productivity and compliance with facility policies
Qualifications
- Open to all experience levels — any experience welcome, including new grads
- Previous experience or comfort working in a Skilled Nursing Facility is a strong fit
- Comfort with floating frequently across units according to facility needs
- Ability to start the 13-week contract with requested start date of 02-09-2026
Knowledge & Skills
- Skilled in occupational therapy evaluations and treatment planning for older adults
- Knowledge of ADL retraining, adaptive equipment, and restorative approaches
- Strong documentation and clinical reasoning skills
- Excellent communication and teamwork when collaborating with interdisciplinary teams
- Time management and adaptability to float and manage a variable caseload
