Roles & Responsibilities

• Deliver skilled physical therapy interventions and assistive techniques to residents in a skilled nursing facility
• Collaborate with nurses, therapists and interdisciplinary team members to create meaningful care plans
• Document resident evaluations, treatment plans and progress notes accurately and timely
• Provide patient and family education on mobility, safety and home exercise programs
• Implement fall prevention and functional mobility programs to improve resident independence
• Communicate effectively with facility leadership about resident needs and care coordination
• Support discharge planning and continuity of care through clear handoffs

Qualifications

• Current or active PTA license in the state of Illinois, or eligibility to obtain licensure
• Graduate of an accredited Physical Therapist Assistant program (associate degree typical)
• Open to clinicians at all experience levels; any experience accepted including new grads
• Comfortable working in a Skilled Nursing Facility practice setting
• Strong communication skills and ability to work as part of an interdisciplinary team

Education

• Graduate of an accredited Physical Therapist Assistant program (associate degree typical)

Knowledge & Skills

• Solid understanding of therapeutic exercise, gait training and mobility assistance
• Experience with documentation and treatment planning in a clinical setting
• Knowledge of SNF workflows, resident-centered care and safety protocols
• Good time management, adaptability and clinical reasoning skills
• Ability to educate residents and families with patience and compassion
