Roles & Responsibilities
- Deliver skilled physical therapy interventions and assistive techniques to residents in a skilled nursing facility
- Collaborate with nurses, therapists and interdisciplinary team members to create meaningful care plans
- Document resident evaluations, treatment plans and progress notes accurately and timely
- Provide patient and family education on mobility, safety and home exercise programs
- Implement fall prevention and functional mobility programs to improve resident independence
- Communicate effectively with facility leadership about resident needs and care coordination
- Support discharge planning and continuity of care through clear handoffs
Qualifications
- Current or active PTA license in the state of Illinois, or eligibility to obtain licensure
- Graduate of an accredited Physical Therapist Assistant program (associate degree typical)
- Open to clinicians at all experience levels; any experience accepted including new grads
- Comfortable working in a Skilled Nursing Facility practice setting
- Strong communication skills and ability to work as part of an interdisciplinary team
Education
- Graduate of an accredited Physical Therapist Assistant program (associate degree typical)
Knowledge & Skills
- Solid understanding of therapeutic exercise, gait training and mobility assistance
- Experience with documentation and treatment planning in a clinical setting
- Knowledge of SNF workflows, resident-centered care and safety protocols
- Good time management, adaptability and clinical reasoning skills
- Ability to educate residents and families with patience and compassion
