Roles & Responsibilities
- Provide skilled physical therapy evaluations and individualized treatment plans for SNF residents
- Deliver hands-on care, therapeutic exercise, and functional training to help patients regain independence
- Collaborate with interdisciplinary team members to coordinate timely, person-centered care
- Document assessments, progress notes, and discharge plans accurately and on time
- Educate patients and families on home exercise programs and safe mobility strategies
- Monitor outcomes and adjust interventions to achieve measurable functional improvements
Qualifications
- Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — state licensure or ability to obtain required MN licensure
- Comfortable working in a skilled nursing facility (SNF) setting
- Clinical skills appropriate for post-acute care and geriatric populations
- Open to all experience levels — any experience accepted, including new graduates
- Strong communication and documentation skills
Education
- DPT preferred or entry-level PT degree (aligned with licensure requirements)
Knowledge & Skills
- Knowledge of post-acute care workflows and SNF documentation expectations
- Skilled in mobility training, therapeutic exercise, balance and gait training
- Ability to assess functional status and set realistic, measurable goals
- Familiarity with interdisciplinary care planning and discharge coordination
- Patient-centered approach with strong clinical reasoning
