Roles & Responsibilities
- Provide compassionate, patient-centered hospice nursing care in home health settings
- Assess symptoms and manage pain using evidence-based approaches and the WHO analgesic ladder
- Administer medications, start and maintain IVs, and perform phlebotomy as needed
- Document thoroughly and accurately using EPIC and HOPE systems
- Educate and support patients and families about goals of care, advanced directives, and symptom management
- Perform death pronouncement and provide postmortem care with dignity and cultural sensitivity
- Collaborate with the interdisciplinary team to coordinate visits and optimize patient comfort
- Manage productivity expectations — typically up to 6 patient visits per day
Qualifications
- Active RN license (Oregon) — pending license accepted
- Minimum 1 year of skilled hospice experience required (first-timers not accepted)
- BCLS certification required
- HOPE documentation experience required — please highlight this on your application
- Own reliable transportation — travel between patient homes expected
- Willingness to meet productivity expectations (standard: ~30 productivity points)
Education
- Active Registered Nurse (RN) license — Oregon (pending accepted)
- BCLS (required)
- Associate Degree in Nursing (ADN) or BSN; BSN preferred but not required
Knowledge & Skills
- Strong hospice and palliative care knowledge: end-of-life care, symptom assessment, cultural awareness
- Proficient with EPIC and HOPE charting systems
- Medication administration, pain management, equianalgesic dosing, and breakthrough dosing
- IV management and phlebotomy skills, including starting and maintaining IVs
- Experience with PCA pump management and pediatric dosage calculations as applicable
- Comfort with death pronouncement, postmortem care, and family support/education
- Familiarity with ethical/legal issues: advanced directives, withholding/withdrawing treatment, organ donation
