Roles & Responsibilities

• Perform telephone triage and follow-up phone calls to ensure patients get timely guidance
• Conduct pain assessments and collaborate on pain management plans
• Handle pharmacy calls and refill requests efficiently and professionally
• Complete medication reconciliation and accurate Epic charting after each encounter
• Monitor and document vital signs and specimen labeling with attention to detail
• Provide compassionate patient and family education around diagnoses and symptom management
• Identify concerning symptoms and escalate appropriately to providers
• Coordinate care and assist with referrals, ensuring smooth outpatient clinic workflows
• Be prepared to float to Centralia if needed and communicate needs with the manager

Qualifications

• Active LPN licensure in Washington (Licensed Practical Nurse)
• Minimum 2 years of clinical nursing experience (onsite outpatient/oncology experience preferred)
• Comfortable with telephone triage, follow-up calls, medication reconciliation and Epic charting
• Locals only — must confirm interest in a temp-to-perm role upon application
• Willing and able to complete an on-camera video interview (please provide 3 available times)
• BLS certification preferred
• Reliable transportation for potential occasional float to Centralia

Education

• LPN diploma or equivalent
• Active LPN license for Washington state

Knowledge & Skills

• Strong skills in telephone triage and patient communication
• Experience with Epic electronic medical record charting
• Knowledge of pain assessment and outpatient symptom management
• Medication reconciliation and handling pharmacy/refill requests
• Ability to label specimens and document patient health history/allergies accurately
• Comfort with outpatient oncology workflows and coordinating services
• Compassionate patient/family education and end-of-life care familiarity
