Roles & Responsibilities
- Provide compassionate hands-on care to residents, assisting with activities of daily living like bathing, dressing, and mobility
- Monitor and record vital signs and report any changes in condition to the nursing team promptly
- Assist with safe patient transfers and positioning to promote comfort and reduce fall risk
- Support the care team with timely, accurate documentation and communication to ensure continuity of care
- Help create a respectful, dignified environment that preserves resident independence and well-being
- Collaborate with nurses and interdisciplinary team members to follow care plans and improve outcomes
Qualifications
- Current Certified Nursing Assistant (CNA) certification required
- T-CNA certification recognized/acceptable as listed
- State nursing assistant registry active and in good standing
- Current BLS/CPR certification preferred or willing to obtain
- Prior experience in long-term care or rehabilitation settings is a plus
- Clear background check and ability to meet facility onboarding requirements
Education
- High school diploma or equivalent
- Completion of an approved Certified Nursing Assistant training program
Knowledge & Skills
- Skilled in personal care, transfers, toileting, and basic patient hygiene
- Ability to take and record vital signs accurately and notice changes
- Familiarity with infection control practices and safe patient handling
- Strong communication skills and teamwork mindset
- Basic charting/documentation skills and attention to detail
- Compassionate bedside manner and respect for resident dignity
