Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living like bathing, dressing, and mobility
• Monitor and record vital signs and report any changes in condition to the nursing team promptly
• Assist with safe patient transfers and positioning to promote comfort and reduce fall risk
• Support the care team with timely, accurate documentation and communication to ensure continuity of care
• Help create a respectful, dignified environment that preserves resident independence and well-being
• Collaborate with nurses and interdisciplinary team members to follow care plans and improve outcomes

Qualifications

• Current Certified Nursing Assistant (CNA) certification required
• T-CNA certification recognized/acceptable as listed
• State nursing assistant registry active and in good standing
• Current BLS/CPR certification preferred or willing to obtain
• Prior experience in long-term care or rehabilitation settings is a plus
• Clear background check and ability to meet facility onboarding requirements

Education

• High school diploma or equivalent
• Completion of an approved Certified Nursing Assistant training program

Knowledge & Skills

• Skilled in personal care, transfers, toileting, and basic patient hygiene
• Ability to take and record vital signs accurately and notice changes
• Familiarity with infection control practices and safe patient handling
• Strong communication skills and teamwork mindset
• Basic charting/documentation skills and attention to detail
• Compassionate bedside manner and respect for resident dignity
