Roles & Responsibilities
- Provide compassionate, hands-on care to residents, assisting with activities of daily living
- Take and record vital signs accurately and report changes to the nursing team
- Support safe transfers and mobility, using proper body mechanics and equipment
- Assist with personal care tasks such as bathing, dressing, and feeding with dignity
- Document resident care and observations clearly and promptly in the chart
- Communicate effectively with nurses, therapists, residents, and families
- Contribute to a positive rehabilitation-focused environment that encourages recovery
- Follow infection control policies and facility safety protocols at all times
Qualifications
- Certified Nursing Assistant (CNA) credential required; NY certification preferred
- Current CPR/BLS certification preferred or willing to obtain
- Ability to pass background checks and required health screenings
- Comfortable working in a long-term care or rehabilitation setting
- Strong communication skills and a compassionate patient-first attitude
- Reliable, punctual, and able to work independently when needed
Education
- High school diploma or GED
- Completion of an accredited CNA training program
Knowledge & Skills
- Hands-on patient care and assistance with ADLs
- Basic vital signs measurement and documentation
- Safe transfer and positioning techniques
- Familiarity with long-term care and rehab routines
- Effective communication and teamwork with interdisciplinary staff
- Basic electronic charting or paper charting skills
