Roles & Responsibilities
- Provide compassionate, person-centered care to residents in the Neighborhoods of Whitelake
- Assist with activities of daily living including bathing, dressing, toileting, and mobility
- Monitor and report changes in resident condition to nursing staff promptly
- Perform routine clinical tasks such as vital signs and basic observations
- Follow individualized care plans and support restorative care goals
- Maintain accurate, timely documentation of care and resident interactions
- Adhere to infection control protocols, vaccine requirements, and facility safety standards
- Participate in competency testing and ongoing training as required
Qualifications
- Active Certified Nursing Assistant (CNA) credential
- BLS certification required
- Successful License/Certificate Verification as part of onboarding
- Completion of Trinity Specialty Specific Competency Testing as required
- Ability to complete required health screenings: Fit Test; 2-step PPD or IGRA; Trinity 10 Panel Drug Screen; Trinity Respirator Evaluation
- Up-to-date immunizations or titers including MMR, Hepatitis B, Varicella, Tdap, COVID-19, and seasonal influenza
- Capacity to complete a physical/health statement and Trinity Color Vision check
Education
- High school diploma or equivalent preferred
- Completed Certified Nursing Assistant (CNA) training program and active certification
Knowledge & Skills
- Strong resident-centered care skills and basic clinical competency
- Ability to take and record vital signs accurately
- Knowledge of infection prevention and PPE best practices
- Clear, compassionate communication with residents, families, and team members
- Good documentation skills and attention to detail
- Teamwork mindset and ability to follow care plans and supervisory direction
- Comfortable working in long-term care or neighborhood-style residential settings
