Roles & Responsibilities
- Provide compassionate personal care and support to patients receiving home care and hospice services
- Assist with activities of daily living including bathing, grooming, mobility assistance, and feeding
- Monitor and record vital signs and changes in patient condition, escalating concerns to the clinical team
- Deliver skilled, dignified end-of-life care that prioritizes comfort and family communication
- Accurately document care activities in patient records and complete required Trinity skills checklist items
- Adhere to infection prevention, safety protocols, and medication administration directions as applicable
- Collaborate with nurses, social workers, and hospice team members to implement individualized care plans
- Maintain required certifications and participate in ongoing training to uphold clinical standards
Qualifications
- Current CNA certification and active state CNA license or eligibility for license verification
- Basic Life Support (BLS) required; NRP and TNCC listed as required certifications for this role
- Experience in home care or hospice settings preferred but caring attitude and strong fundamentals are essential
- Successful completion of Trinity Skills Checklist and license/certificate verification
- Must meet health screening requirements: fit test, 2-step PPD or IGRA, MMR, Hepatitis B, Varicella, Tdap, physical/health statement, COVID-19 vaccine, influenza vaccine
- Negative Trinity 10-panel drug screen as part of onboarding
Education
- High school diploma or equivalent required; CNA certification required
- Additional hospice or palliative care training is a plus
Knowledge & Skills
- Strong patient-centered care and interpersonal communication skills
- Comfort with activities of daily living, safe patient transfers, and mobility assistance
- Basic clinical skills including vital signs, observing clinical changes, and documentation
- Familiarity with hospice philosophy, comfort care principles, and family support
- Ability to follow infection control practices and comply with health screening requirements
- Teamwork and reliable time management for per diem/pool assignments
