Roles & Responsibilities
- Provide compassionate, patient-centered care in home and hospice settings, honoring patient dignity and comfort
- Assist with activities of daily living such as bathing, grooming, mobility support, and feeding with attentiveness and respect
- Perform basic clinical tasks as directed and document care accurately and timely in the chart
- Communicate changes in patient condition promptly to the care team and participate in care planning discussions
- Support family members and caregivers with education, emotional support, and practical guidance
- Adhere to infection control, safety policies, and all required screening and competency checks
Qualifications
- Active CNA certification in the state of Michigan (required)
- Basic Life Support (BLS) certification (required)
- License/certificate verification and successful completion of Trinity Specialty Specific Competency Testing
- Current fit test / respirator evaluation and Trinity Color Vision as required
- Up-to-date immunizations and health screenings: MMR, Hepatitis B, Varicella, Tdap, COVID-19, Influenza, and 2-step PPD or IGRA
- Clean 10-panel drug screen and physical/health statement
- Previous experience in home care or hospice preferred but caring attitude and strong communication skills are essential
Education
- CNA certification (required)
- High school diploma or equivalent (preferred)
Knowledge & Skills
- Knowledge of home care and hospice best practices and comfort providing end-of-life support
- Strong interpersonal skills and ability to build rapport with patients and families
- Good clinical observation and documentation skills
- Ability to follow care plans and take direction from interdisciplinary teams
- Time management and flexibility to work varied shifts in a per diem pool
- Familiarity with infection control, safety standards, and respirator procedures
