Roles & Responsibilities

• Provide compassionate direct patient care, assisting with activities of daily living
• Monitor and record patient vital signs and report changes to the nursing team
• Perform safe patient transfers and mobility assistance following facility protocols
• Maintain clear, accurate documentation of care and Trinity checklist items
• Adhere to infection control policies and complete required fit testing when assigned
• Collaborate with nurses and interdisciplinary staff to support high-quality care
• Complete required skills evaluations and comply with license/cert verification processes

Qualifications

• Current CNA certification in the state of Connecticut (or eligible for verification)
• BLS (Basic Life Support) certification required
• Successful completion of Trinity Skills Checklist and License/Certificate Verification
• Must complete required screenings and health checks: 2-step PPD or IGRA (Quantiferon), Trinity 10-panel drug screen, fit test, respirator evaluation
• Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine and annual influenza vaccine
• Ability to pass facility onboarding requirements and comply with per diem pool policies

Education

• High school diploma or equivalent
• Current Certified Nursing Assistant (CNA) credential

Knowledge & Skills

• Skilled in basic patient care: ADLs, hygiene, feeding and mobility assistance
• Knowledge of infection prevention and proper PPE use
• Competent at taking and documenting vital signs and observing patient condition
• Good communication and teamwork skills to collaborate with nurses and staff
• Comfort with completing skills checklists and following standardized care protocols
