Roles & Responsibilities
- Provide compassionate direct patient care, assisting with activities of daily living
- Monitor and record patient vital signs and report changes to the nursing team
- Perform safe patient transfers and mobility assistance following facility protocols
- Maintain clear, accurate documentation of care and Trinity checklist items
- Adhere to infection control policies and complete required fit testing when assigned
- Collaborate with nurses and interdisciplinary staff to support high-quality care
- Complete required skills evaluations and comply with license/cert verification processes
Qualifications
- Current CNA certification in the state of Connecticut (or eligible for verification)
- BLS (Basic Life Support) certification required
- Successful completion of Trinity Skills Checklist and License/Certificate Verification
- Must complete required screenings and health checks: 2-step PPD or IGRA (Quantiferon), Trinity 10-panel drug screen, fit test, respirator evaluation
- Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine and annual influenza vaccine
- Ability to pass facility onboarding requirements and comply with per diem pool policies
Education
- High school diploma or equivalent
- Current Certified Nursing Assistant (CNA) credential
Knowledge & Skills
- Skilled in basic patient care: ADLs, hygiene, feeding and mobility assistance
- Knowledge of infection prevention and proper PPE use
- Competent at taking and documenting vital signs and observing patient condition
- Good communication and teamwork skills to collaborate with nurses and staff
- Comfort with completing skills checklists and following standardized care protocols
