Roles & Responsibilities
- Provide compassionate direct patient care in the home environment, assisting with activities of daily living
- Perform clinical tasks within CNA scope, including vital signs, personal care, and mobility support
- Assist with safe transfers and fall prevention measures to protect patients
- Document patient status and care delivered accurately and promptly using required systems
- Communicate clearly with patients, families, and the care team to support coordinated care
- Adhere to infection control policies, safety protocols, and Trinity clinical checklists
- Participate in required training, skills checklists, and competency verifications
Qualifications
- Current CNA certification in good standing
- BLS (Basic Life Support) certification required
- Comfortable working in home health settings; home health experience is a plus
- Successful completion of Trinity Skills Checklist and license/certificate verification
- Ability to pass required health screenings and immunizations (see checklist)
- Willingness to comply with credentialing requirements and periodic testing
Education
- CNA certification required
- High school diploma or equivalent preferred
Knowledge & Skills
- Strong hands-on patient care skills and knowledge of ADLs
- Familiarity with basic vital sign measurement and documentation
- Infection prevention and control practices
- Good communication and interpersonal skills to engage patients and families
- Able to follow care plans and report changes in patient condition
- Comfortable working independently in patients' homes
