Roles & Responsibilities

• Provide compassionate direct patient care in the home environment, assisting with activities of daily living
• Perform clinical tasks within CNA scope, including vital signs, personal care, and mobility support
• Assist with safe transfers and fall prevention measures to protect patients
• Document patient status and care delivered accurately and promptly using required systems
• Communicate clearly with patients, families, and the care team to support coordinated care
• Adhere to infection control policies, safety protocols, and Trinity clinical checklists
• Participate in required training, skills checklists, and competency verifications

Qualifications

• Current CNA certification in good standing
• BLS (Basic Life Support) certification required
• Comfortable working in home health settings; home health experience is a plus
• Successful completion of Trinity Skills Checklist and license/certificate verification
• Ability to pass required health screenings and immunizations (see checklist)
• Willingness to comply with credentialing requirements and periodic testing

Education

• CNA certification required
• High school diploma or equivalent preferred

Knowledge & Skills

• Strong hands-on patient care skills and knowledge of ADLs
• Familiarity with basic vital sign measurement and documentation
• Infection prevention and control practices
• Good communication and interpersonal skills to engage patients and families
• Able to follow care plans and report changes in patient condition
• Comfortable working independently in patients' homes
