Roles & Responsibilities
- Provide compassionate personal care and assistance with activities of daily living
- Support patients in a home environment with bathing, grooming, mobility, and feeding
- Take and record vital signs and observe changes in patient condition
- Maintain accurate, timely documentation in the patient record
- Follow infection control practices and facility-specific safety protocols
- Complete Trinity specialty competency testing and demonstrate required skills
- Communicate effectively with nurses, therapists, and family members about patient needs
- Respect patient dignity and promote independence while delivering skilled care
Qualifications
- Current CNA certification or state-approved nursing assistant credential
- Basic Life Support (BLS) certification required
- License/certificate verification will be completed prior to assignment
- Must meet health requirements including 2-step PPD or IGRA, required immunizations (MMR, Hepatitis B, Varicella, Tdap), COVID-19 vaccine, and annual influenza vaccine
- Successful completion of Trinity 10 panel drug screen and respirator evaluation / fit test
- Willingness to complete Trinity specialty-specific competency testing
- Professional, dependable, and patient-centered with good communication skills
- Availability to work per diem/pool shifts as needed
Education
- High school diploma or GED preferred
- Completion of an approved CNA training program and current CNA certification
Knowledge & Skills
- Strong skills in assisting with ADLs and safe patient handling
- Knowledge of infection prevention and standard precautions
- Comfortable with basic clinical observations and documentation
- Ability to follow care plans and report changes promptly
- Good time management and ability to work independently in patient homes
- Empathetic, patient-focused bedside manner and strong interpersonal skills
- Familiarity with home health safety considerations and home environment assessments
