Roles & Responsibilities
- Provide direct personal care and assistance to residents with dignity and respect
- Assist with activities of daily living including bathing, dressing, toileting, and mobility
- Take and record vital signs and report changes in condition to nursing staff
- Follow individualized care plans and contribute observations to the clinical team
- Participate in mandatory competency testing and skills checklists to maintain high-quality care
- Maintain infection control practices and adhere to facility health requirements
- Support a compassionate, person-centered environment that promotes resident independence
Qualifications
- Current Certified Nursing Assistant (CNA) certification or state-equivalent (license/certification verification required)
- Basic Life Support (BLS) certification required
- Willingness to complete Trinity specialty competency testing and Trinity skills checklist
- Successful completion of required health screenings: 2-step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, physical/health statement, COVID-19 vaccine, and annual influenza vaccine
- Clear Trinity 10-panel drug screen and color vision check as part of onboarding
- Reliable communication skills and ability to work collaboratively with nursing staff
Education
- Completion of a state-approved CNA training program or equivalent (high school diploma or GED preferred)
Knowledge & Skills
- Patient-centered care and safe transfer/mobility assistance
- Accurate vital sign measurement and documentation
- Basic infection prevention and PPE use
- Ability to follow care plans, checklists, and competency protocols
- Professionalism, strong interpersonal skills, and teamwork
- Comfort working in community-based/longitudinal care settings
