Roles & Responsibilities
- Provide compassionate, hands-on personal care to residents including bathing, grooming, and mobility assistance
- Monitor and record vital signs and resident conditions, escalating concerns to nursing staff promptly
- Support activities of daily living while promoting independence and dignity
- Complete required documentation accurately and timely to support continuity of care
- Assist with safe transfers and positioning to prevent injury and maintain comfort
- Participate in mandatory competency testing and skills checklists to ensure high-quality care
- Work collaboratively with interdisciplinary team members to deliver resident-centered care
Qualifications
- Valid CNA license/certification as required by the facility
- BLS certification (required)
- Successful completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Ability to pass license and certificate verification
- Current immunizations and health requirements: 2 Step PPD or IGRA (Quantiferon), MMR, Hepatitis B, Varicella, Tdap, COVID-19 vaccine, and annual influenza vaccine
- Successful Trinity 10 panel drug screen and physical/health statement
Knowledge & Skills
- Strong resident-focused bedside care skills
- Knowledge of infection control practices and standard precautions
- Comfortable with documentation in clinical charts and shift reports
- Good communication and teamwork skills with residents, families, and staff
- Ability to follow care plans and report changes in condition
- Competency in safe patient handling and mobility support
