Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately five meaningful patient visits each day using your clinical expertise
Document care accurately with the provided tablet and HomeCare HomeBase EMR system
Collaborate with the intake RN and manager to coordinate seamless patient support

Qualifications

Active Physical Therapist license in California
BLS certification required
Home Health experience preferred but candidates with less than two years will be considered if they have Home Health experience
Comfortable traveling within the service area and managing a daily mileage of around 40 miles

Knowledge & Skills

Physical Therapy evaluation and treatment planning
Home Health documentation and compliance
Orthopedic and geriatric patient care expertise
Strong communication and patient education skills
Ability to work independently in home settings
