Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate care coordination for patients in need of mental health support
Conduct thorough assessments and develop individualized care plans
Collaborate with interdisciplinary teams to ensure seamless patient care
Support patients and families through crisis situations with empathy and expertise
Maintain accurate documentation while building meaningful connections with those you serve

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities strongly preferred
Strong assessment and crisis intervention skills

Knowledge & Skills

Expertise in involuntary psychiatric holds and mental health crisis intervention
Understanding of California mental health regulations
Ability to work collaboratively within multidisciplinary teams
Strong communication and documentation abilities
Compassionate approach to patient advocacy
