Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct comprehensive Oasis Assessments and Start of Care evaluations
Develop personalized treatment plans for elderly patients recovering from falls and fractures
Track visits and time using tablet technology for seamless documentation
Deliver 4-6 meaningful patient visits daily while maintaining quality care standards

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural community settings
Strong assessment and clinical reasoning skills

Knowledge & Skills

Oasis Assessment documentation
Start of Care evaluation experience
Rural healthcare delivery
Geriatric rehabilitation
Tablet-based documentation systems
