Roles & Responsibilities

• Evaluate and treat patients in a skilled nursing setting to improve communication and swallowing function
• Develop individualized treatment plans and document progress in accordance with facility standards
• Collaborate closely with nursing and rehab team members to coordinate high-quality care
• Maintain productivity targets and follow CRG policies and procedures
• Participate in care planning and discharge coordination to support positive patient outcomes
• Represent the therapy department professionally (dress code: black scrubs) and be present on-site as required

Qualifications

• 1-3 years of related SLP experience (as listed in the order)
• Active SLP credentials/licensure as required by state and facility (confirm state requirements prior to start)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Ability to meet productivity expectations (90%) and follow facility/CRG policies
• Professional appearance and ability to be on-site to receive guaranteed hours

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy practices and documentation
• Assessment and treatment of adult communication and swallowing disorders
• Interdisciplinary teamwork and effective communication with care teams
• Familiarity with productivity tracking and meeting contractual productivity targets
• Adherence to policies and procedures and ability to work within established clinical guidelines
