Roles & Responsibilities

• Coordinate IDT-approved meal deliveries, durable medical equipment, incontinence supplies, and caregiver services to ensure participants receive timely, person-centered care
• Conduct pre-enrollment, annual, semi-annual, change-of-address, and monthly home safety evaluations and document findings
• Complete and update Home Care Coordination care plans at enrollment and semi-annually, collaborating with the interdisciplinary team to align care with participant goals
• Monitor service delivery, review progress notes, and flag gaps in care for the IDT to reconcile and resolve
• Document and upload reports, progress notes, and vendor/ care-related documentation into the EMR accurately and timely
• Communicate regularly and compassionately with participants, families, caregiver agencies, DME suppliers, and healthcare providers to coordinate schedules and ensure quality
• Support PACE nurses and Home Care Coordination RNs as requested, including conducting home visits and delivering supplies or equipment
• Participate in staff meetings, in-service education, and interdisciplinary team meetings while maintaining HIPAA and regulatory compliance

Qualifications

• Valid CNA certificate
• California Department of Public Health Home Health Aide (HHA) certificate
• BLS / CPR and First Aid certification from the American Heart Association
• Minimum one year of CNA experience working with the frail or elderly population
• Valid California Driver's License
• Ability to pass pre-employment I-9, physical, background and reference checks, PPD screening, and required vaccinations

Education

• CNA certificate
• California Department of Public Health Home Health Aide (HHA) certificate
• BLS / CPR and First Aid (American Heart Association)

Knowledge & Skills

• Strong communication, teamwork, coordination, and evaluation skills
• Basic computer skills and comfort with EMR documentation; experience with Microsoft Outlook and Excel
• Knowledge of Home Health Aide scope of practice and ability to work within scope
• Familiarity with HIPAA, OSHA, infection control precautions, and regulatory compliance (DHCS/CMS)
• Good organizational skills, attention to detail, and customer service orientation
