Roles & Responsibilities

• Assess patient mobility, function, and needs to create individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve strength, balance, and mobility
• Document patient progress and update care plans in the medical record accurately and timely
• Collaborate with interdisciplinary team members to optimize patient outcomes and discharge planning
• Educate patients and families on home exercise programs and safety strategies to support recovery
• Monitor and modify treatment based on patient response and clinical judgment

Qualifications

• Minimum 1 year of experience in Physical Therapy (explicitly required)
• Valid Physical Therapist license in the state of practice (or ability to obtain) preferred
• Current BLS/CPR certification preferred
• Strong communication and teamwork skills with a patient-centered approach

Education

• Degree from an accredited Physical Therapy program (as typical for the role)

Knowledge & Skills

• Physical Therapy clinical assessment and treatment
• Therapeutic exercise and mobility training
• Patient documentation and clinical charting
• Interdisciplinary communication and care coordination
• Basic knowledge of safety and fall-prevention strategies
