Roles & Responsibilities

• Provide compassionate day-to-day nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and document per facility policies
• Perform assessments, monitor vital signs, and report changes in patient condition
• Collaborate with RNs, therapists, and interdisciplinary team members to support care plans
• Assist with admissions, transfers, and safe patient handling to ensure dignity and safety
• Educate residents and families on care plans and discharge/aftercare instructions
• Maintain accurate, timely documentation in the patient record and follow facility protocols

Qualifications

• Active LPN license for New Mexico (required)
• Recent LPN experience in long-term care, skilled nursing, or rehabilitation preferred
• BLS/CPR certification (or willingness to obtain upon hire)
• Current immunizations and ability to pass background and drug screening
• Strong communication, clinical judgement, and patient advocacy skills
• Ability to work Monday-Friday daytime hours as scheduled

Education

• LPN vocational diploma or equivalent; current New Mexico LPN licensure required

Knowledge & Skills

• Medication administration and basic IV/SC skills as required by site protocols
• Wound care basics and assistance with dressing changes
• Familiarity with documentation standards and electronic medical records
• Infection control practices and safe patient handling
• Ability to prioritize tasks in a fast-paced long-term care environment
• Excellent interpersonal skills for resident-centered care and family interactions
