Roles & Responsibilities

• Provide safe, compassionate patient care as a Licensed Practical Nurse, delivering day-to-day treatments and monitoring
• Administer medications and treatments accurately while following facility policies and protocols
• Perform timely and thorough patient assessments and report changes to the care team
• Document care and patient responses clearly and professionally in the medical record
• Collaborate with nurses, therapists, and support staff to create and execute patient-focused care plans
• Support patients and families with clear communication, education, and empathy

Qualifications

• Current and active LPN license (Licensed Practical Nurse) — required
• Must be local or able to reliably commute (this is a LOCAL ONLY assignment)
• Comfortable working the scheduled days (Fri, Sat, Sun) and associated shift times
• Strong communication, teamwork, and documentation skills
• CPR/BLS certification preferred or willing to obtain if not current

Education

• LPN diploma or vocational nursing certificate and active state LPN licensure

Knowledge & Skills

• Solid knowledge of medication administration and basic nursing procedures
• Ability to perform patient assessments and recognize changes in condition
• Competence with electronic charting and accurate documentation
• Infection control, safety practices, and adherence to facility protocols
• Strong interpersonal skills and ability to work effectively within a multidisciplinary team
