Roles & Responsibilities

• Coordinate patient care across teams to ensure smooth transitions and timely discharges
• Perform thorough assessments and develop individualized care plans in collaboration with providers
• Manage case coordination activities and follow-up for a diverse admitting diagnosis population
• Document care accurately and efficiently using Cerner to maintain continuity and compliance
• Communicate clearly with patients, families, hospitalists, residents, and support staff
• Educate patients and families on care plans, resources, and discharge instructions
• Supervise and collaborate with care assistants to optimize patient workflows and outcomes
• Prioritize caseloads and advocate for patient needs to improve quality and safety

Qualifications

• Active California Registered Nurse license required
• BLS certification required
• Minimum 2 years of RN experience (case management or acute care experience preferred)
• Experience with care coordination/ case management workflows
• Familiarity with Cerner charting system
• Ability to work day shift schedule (M-F 8:00 AM - 4:30 PM) with occasional weekends
• Professional appearance and strong interpersonal/communication skills
• Comfort collaborating with hospitalists, residents, and interdisciplinary teams

Education

• BSN: Not required (position indicates BSN not required); active RN licensure in California is required
