Roles & Responsibilities

Evaluate residents and develop individualized occupational therapy treatment plans that promote independence
Provide focused therapy for activities of daily living (ADLs), mobility, and functional skills
Implement therapeutic exercises and activities to improve coordination and upper extremity function
Monitor resident progress and update treatment plans to reflect outcomes and goals
Collaborate with physicians, therapists, nurses, and family members to create cohesive care plans
Maintain accurate, timely clinical documentation and progress notes
Educate residents and caregivers on adaptive techniques and safety strategies to enhance daily living

Qualifications

Active California Occupational Therapist license (required)
Degree in Occupational Therapy from an accredited program (required)
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong clinical assessment and patient care skills
Ability to work effectively within a multidisciplinary healthcare team
Excellent communication and documentation abilities

Education

Degree in Occupational Therapy from an accredited program

Knowledge & Skills

Expertise in ADL training and functional rehabilitation
Patient evaluation and individualized treatment planning
Therapeutic exercise prescription and progress monitoring
Effective clinical documentation and charting
Strong interpersonal skills for working with residents, families, and care teams
