Physical Therapy Assistant - SNF

Location: Watsonville, CA

Candidate Pay Rate: $68 - $47.6 per hour

Roles & Responsibilities

Assist in implementing treatment plans developed by the supervising Physical Therapist to help residents regain independence
Provide therapy focused on mobility, strength, balance, and functional tasks to improve quality of life
Guide residents through therapeutic exercises and activities with compassion and clear instruction
Monitor patient progress closely and report changes promptly to the supervising Physical Therapist
Maintain accurate and timely therapy documentation to ensure continuity of care
Collaborate with interdisciplinary care teams to coordinate comprehensive rehabilitation plans
Educate residents and families on safe mobility techniques and fall prevention strategies

Qualifications

Active California Physical Therapy Assistant (PTA) License required
Associate Degree from an accredited PTA program required
Previous SNF, rehabilitation, or geriatric therapy experience preferred
Strong patient care mindset with excellent communication skills
Ability to work effectively within a multidisciplinary healthcare team

Education

Associate Degree from an accredited PTA program

Knowledge & Skills

Experience supporting mobility training and rehabilitation programs
Strong observation and patient monitoring abilities
Effective documentation and record-keeping skills
Clear, compassionate communication with residents, families, and care teams
Comfortable working in a skilled nursing facility environment
