Roles & Responsibilities

• Assess patients’ functional abilities and occupational needs to create meaningful goals
• Develop individualized treatment plans that promote independence and quality of life
• Implement evidence-based OT interventions and adapt them as patients progress
• Document thoroughly and accurately in EPIC to ensure continuity of care
• Collaborate with interdisciplinary teams, families, and caregivers to coordinate care
• Participate in Trinity specialty competency testing and skills checklists to maintain high standards
• Maintain required licenses, certifications, and health screenings to meet facility standards
• Communicate progress and discharge planning clearly with patients and the care team

Qualifications

• Valid Occupational Therapy license (Michigan) — license/certificate verification required
• BLS (Basic Life Support) certification required
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Ability to complete required health screenings (drug screen, immunizations, fit test, etc.)
• Experience documenting in EPIC and working within interdisciplinary clinical teams

Knowledge & Skills

• Clinical occupational therapy assessment and treatment planning
• Proficient EPIC documentation and electronic health record workflows
• Strong patient-centered communication and education skills
• Familiarity with Trinity competency standards and skills verification
• Good clinical judgment, organization, and teamwork
