Speech-Language Pathologist - Skilled Nursing Facility
Location: San Rafael, CA
Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities
- Evaluate residents and design individualized speech, language, cognitive, and swallowing treatment plans that make a real difference
- Diagnose and implement therapeutic interventions to improve communication and safe swallowing
- Monitor resident progress, adjust plans of care, and document outcomes clearly and timely
- Collaborate with physicians, nursing, and therapy colleagues to coordinate person-centered care
- Educate residents and caregivers on communication strategies and swallowing safety to support independence
- Maintain accurate clinical documentation and contribute to a positive, outcomes-driven rehab environment
Qualifications
- Active California Speech-Language Pathologist license
- Master’s Degree in Speech-Language Pathology from an accredited program
- Certificate of Clinical Competence (CCC-SLP) preferred
- Previous SNF, rehabilitation, or geriatric therapy experience preferred
- Strong clinical assessment, treatment planning, and documentation skills
- Excellent communication and interdisciplinary collaboration abilities
Education
- Master’s Degree in Speech-Language Pathology from an accredited program
Knowledge & Skills
- Expertise in speech, language, cognitive, and swallowing therapy
- Skilled in patient evaluation and individualized treatment planning
- Proficient in clinical documentation and progress monitoring
- Strong interpersonal skills for working with residents, families, and care teams
- Familiarity with geriatric rehab practices and SNF workflows
