Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans for home health patients
• Deliver hands-on therapy and skilled interventions to improve mobility, strength, and function
• Document patient encounters clearly and timely in the electronic health record (Epic preferred)
• Educate patients and caregivers on exercise programs, safe ambulation, fall prevention, and home safety
• Coordinate care with nurses, aides, physicians, and other members of the care team to achieve patient goals
• Complete required competency testing and maintain certifications as specified by Trinity
• Ensure compliance with infection control and required immunizations during patient visits

Qualifications

• Active Physical Therapy license or eligibility for state licensure (license verification required)
• BLS certification (required)
• Home health experience preferred; strong clinical assessment and intervention skills
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist as required
• Current immunizations and clearance per employer policy (MMR, Hep B, Varicella, Tdap, COVID-19, Influenza, 2-step PPD or IGRA, drug screen, fit test, color vision)
• Strong communication skills and ability to work independently in patients' homes

Knowledge & Skills

• Home health best practices and safety for in-home care
• Skilled documentation and clinical charting (Epic preferred)
• Patient education techniques and caregiver training
• Therapeutic exercise, gait training, balance and fall prevention strategies
• Care coordination and interdisciplinary collaboration
• Infection control and adherence to required health screenings and vaccinations
