Location: Sunnyvale, California

Start Date: 06-15-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:30 PM (Schedule can be flexible based on patient's availability)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Restore function and prevent disability following disease or injury
Help clients reach their maximum performance within their capabilities
Evaluate and treat patients using medically prescribed physical therapy programs
Supervise Physical Therapy Assistants as needed
Conduct initial admission and evaluation visits
Document observations, clinical findings, skilled interventions, goals, and discharge plans
Notify Clinical Team Manager of OASIS scores within established timelines

Qualifications

Active Physical Therapist license in California
BLS/BCLS certification required
One year of Home Health experience required
Strong documentation and clinical assessment skills

Knowledge & Skills

Physical Therapy evaluation and treatment techniques
Home Health OASIS documentation
Patient-centered care planning
Supervision of Physical Therapy Assistants
Clinical observation and progress documentation
