Location: Valley City, North Dakota

Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide personalized physical therapy services to 5-7 patients daily in their homes
- Help patients recover from surgeries, falls, and other conditions with tailored treatment plans
- Support both Home Health and Hospice patients on their wellness journeys
- Document care accurately using the HomeCare/HomeBase EMR system
- Travel within a 60-mile radius of Valley City to deliver compassionate, one-on-one care
Qualifications
- Current North Dakota Physical Therapy License
- BLS certification required
- At least 1 year of previous home health experience
- Valid driver's license and auto insurance
- Strong PT-related clinical skills
Knowledge & Skills
- Home health physical therapy expertise
- Familiarity with HomeCare/HomeBase EMR
- Ability to manage a caseload of 5-7 patients per day
- Comfort working independently in home settings
- Excellent time management and organizational abilities
