Roles & Responsibilities

• Provide skilled nursing and compassionate case management for patients receiving hospice care in their homes
• Coordinate comprehensive care plans through an interdisciplinary team approach to ensure continuity and quality outcomes
• Assess, evaluate, and monitor patients' physical and emotional well-being while adjusting care as needed
• Educate and support patients and families on symptom management, medications, and end-of-life planning
• Oversee care provided by Home Health Aides and CNAs to maintain high standards and compliance with care plans
• Maintain accurate documentation and patient records in line with agency and regulatory standards
• Participate in on-call rotations and respond to patient needs via phone or in-person visits as required

Qualifications

• Current, active RN license in the State of Montana
• Minimum 2 years of recent acute care, home care, or hospice experience
• Valid driver's license, reliable transportation, and appropriate auto insurance
• HCHB experience strongly preferred
• BSN preferred but not required

Knowledge & Skills

• Strong assessment and care planning abilities
• Skilled in venipuncture, catheterization, wound care, and parenteral therapy
• Excellent communication and coordination with interdisciplinary teams
• Knowledge of OASIS documentation and current reimbursement guidelines
• Compassionate approach to patient and family education
