Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across greater Sacramento county
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
- Help patients regain mobility and independence in the comfort of their own homes
- Collaborate with interdisciplinary teams to deliver exceptional patient-centered care
- Document patient progress and maintain accurate clinical records
- Build meaningful relationships with patients and their families while supporting their recovery journey
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum experience requirements apply - new grads considered only in exceptional cases
- Strong clinical skills across multiple specialties including cardio, pulmonary, orthopedic, neurological, and geriatric care
- Excellent communication and interpersonal abilities
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Home health physical therapy experience
- Ability to work independently in various home settings
- Strong organizational and time management skills
- Compassionate bedside manner with geriatric populations
- Proficiency in electronic medical records documentation
