Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover from joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful visits each day using your clinical expertise
Document care accurately using the HomeCare HomeBase EMR system on your provided tablet
Collaborate with the intake RN and manager to coordinate excellent patient outcomes

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
Strong assessment and documentation skills
Ability to work independently while providing compassionate care

Knowledge & Skills

Physical Therapy evaluation and treatment planning
Home Health documentation and compliance
Orthopedic rehabilitation and geriatric care
Patient education and family communication
Comfortable with EMR systems and tablet-based documentation
