Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to 5-7 patients daily in their homes
Help patients recover from surgeries, falls, and other conditions with personalized care plans
Support both home health and hospice patients on their wellness journeys
Document care accurately using HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver consistent, high-quality therapy

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Knowledge & Skills

Home health and hospice patient care expertise
Comfortable managing a variety of diagnoses including post-surgical and fall recovery
Proficient with HomeCare/HomeBase EMR documentation
Excellent time management for a 5-7 patient daily caseload
Independent worker comfortable with minimal on-site supervision
